The following form should be completed by all International Students. The Flight School of Gwinnett will use this information to process the student's Form I-20 and other documents required by US Immigration. If you have any questions or statements that The Flight School may help you with, please include them at the end of the Enrollment Form.

INTERNATIONAL STUDENTS NOTE: Your name must appear exactly as it is published on your passport. Any misspellings or changes will void your form I-20. There are no exceptions!
